HOW DO I....

OBTAIN A REPEAT PRESCRIPTION?

 Telephone requests are no longer available.

If you require repeat prescriptions you can order your medication by either:-

  1. Ordering the repeat prescription from reception desk, or

  2. Sending a stamped addressed envelope with your computer request which will be returned to yourself or a chemist of your choice.

If you have a query regarding your prescription please ring between 12.30 - 2.00pm.

Your prescriptions will normally be available 48 hours following the request.

HOW DO I...
OBTAIN A SICKNESS / ABSENCE CERTIFICATE?

Self certification forms for the first six days of illness are obtainable as follows:-

  1. SC2 (for employed) from your employer or DSS.

  2. SC1 (for self employed or unemployed) from the DSS or the surgery.

  3. Continuation certificate to be requested after 2.00pm daily.

If you are sick longer than this you must see the doctor to obtain a sickness medical certificate. Private health certificates for which a fee is payable are still issued if needed. Please ask the receptionist.
